1. Can a spreadsheet be attached rather than completing the Schedule?

Page 1 of Schedule SSA must be attached to any spreadsheet containing the participant data to be reported. The spreadsheet must present the data in the same columnar format as the actual Schedule. Page 1 of Schedule SSA must be signed by the plan administrator.


2. Tip: Reconcile the number of individuals list on Schedule SSA for any reason with the count reported on Form 5500 at line 7i.
